Certificates of Insurance
Certificates of insurance are usually needed when dealing with an outside vendor or facility. To obtain a certifcate of insurance complete a Certificate of Insurance Request Form and submit the form to the Office of Risk Management together with pertinent insurance documents. Standard turn-around time is 3 to 5 business days. If the certificate is for an event submit your request 2 weeks prior to the event.
Respond to all liability claims: Property, Automobile, General Liability, Professional Liability (Medical Malpractice), Educators Legal Liability, Workers Compensation, Student Accident, and any other liability claims.
Work with colleges and departments to resolve insurance and idemnification issues in leases and agreements with third-party organizations and vendors. Colleges and departments should consult with University Risk Management for review of any contracts containing insurance provisions, waivers of subrogation, additional insured status, liability limitations or waivers, indemnification, or hold harmless clauses.
Policies and Procedures
Interpret and make recommendations with respoect to the application of risk management and insurance policies and procedures. Consult with and offer guidance to colleges and departments about risk management issues.
Release of Liability and Waiver Forms
The Office can prepare Release of Liability and waiver forms for your college or department, and can discuss your liability concerns for events and/or activities.
Make recommendations with respect to on campus or off campus activities and the use of facilities by non-University individuals and/or groups.
