Application Process (Class Entering fall 2008)

For students with advanced standing, please see the application process for the Transitional DPT Program.

Application Deadline

All applications must be postmarked on or before December 1, 2007. Applications postmarked after December 1, 2007 will be considered on a space-available basis. We encourage students with complete application materials to apply early. Priority consideration will be given to those applicants who have an overall and prerequisite GPA of 3.5 or greater, and have no more than two prerequisites in progress and whose application is postmarked on or before October 15, 2007.

Use the following links to download and print the necessary application forms. The forms are provided in PDF format and can only read using Acrobat Reader. To download a free copy, visit the Plug-Ins page. The application is a fill-in PDF form. Simply download the application to your desktop and type in your information. The application is also available by mail. Please contact the Admissions Office at (909) 469-5335 or admissions@westernu.edu to request one.

Important Notes and Warnings

  • The information entered into the PDF forms below cannot be saved! Do not close the forms until you are completely done and satisfied with the printed result. Leave the forms open and complete each in one sitting.
  • You may print blank copies of all forms and complete your first draft by hand.

Instructions

Please carefully follow the steps outlined below.

STEP 1: Download the latest version of Adobe Acrobat by clicking here. Reboot your computer after installing Adobe Acrobat Reader.

NOTE: Your browser must be one of the following: Internet Explorer 4.x and above or Netscape 6.x.

STEP 2: Download and save the PDF forms on your computer:

Here are some easy steps to save a PDF form on your computer:
1) Right click on the PDF link
2) Select 'Save Target as'
3) Choose a directory where you will save the PDF file (your desktop is a good spot for easy access) 4) Select 'Save' to save the file on your computer.

Tips for navigating the PDF forms:
1) TAB key moves you from one field to another

2) SPACE BAR selects a Yes/No field

STEP 3: Open and print the Application Checklist and use as a guide in the organization of your application packet. You may also wish to print blank copies of all forms and complete your first draft by hand.

STEP 4: Open and complete the Application for Admission that you just saved on your computer.

STEP 5: Print the completed Application for Admission and proofread. Make any necessary adjustments and print your revised final draft.

STEP 6: Open and print two copies of the Work-Volunteer Experience form. Complete one for each facility you worked in for at least 200 hours. Remember that we require you to work or volunteer in at least two facilities and/or clinical settings.

STEP 7: Open and print three copies of the Recommendation Form that you just saved on your computer. Please read and sign the waiver section on each evaluation, and distribute one form to each of your three evaluators/references. You must collect hard copy completed evaluations from each reference for inclusion in your single application packet (see Application Checklist for instructions).

Submitting Your Application

  • All Checklist materials must be included, as requested, in the single application packet submitted by you to Western University of Health Sciences.
  • Retain copies of all submitted materials for your records, as applications become the property of Western University of Health Sciences upon submission.

Interviews

On-campus interviews are not required. However, prospective applicants are encouraged to attend one of our DPT Information Sessions. These sessions are offered throughout the year at our Pomona, California campus. You are also invited to make an appointment with the Physical Therapy department at any time to meet with faculty, speak with students, sit in to observe a class, and tour our facilities. Applicants with Foreign Course Work

Applicants who wish to use course work completed outside the United States must submit their transcripts for evaluation to a Western University Approved Service.

A course-by-course evaluation is required and all course work must be designated as undergraduate, graduate or professional. Western University will only honor evaluations from one of the above services. The evaluation must be included with the application packet.
