How to Apply to the Program

The 6th grade academies are open to any PUSD 6th grade student. Please select one of the links to fill out the RSVP/Release form in English or Spanish.

The PHCL Medical Scholars program is open to students in PUSD starting in the 7th grade. An informational application form, a personal statement of career interests, two letters of recommendation from PUSD staff (teachers, principals, tutors) and an in-person discussion for students and parents with members of the Health Career Ladder will be required.

Students who have attended the 6th grade academies are especially encouraged to apply. Interviews will be conducted in September-October of 2009.
