This form is available at the Office of the Registrar. Please include all of the following information:<\/p>\n<ul>\n<li>Full legal name (include any former names used while attending)<\/li>\n<li>Social Security number or student ID number<\/li>\n<li>Program and year of graduation (if applicable)<\/li>\n<li>Current address and daytime telephone number<\/li>\n<li>Date of request<\/li>\n<li>Requests\u00a0<strong><em>must\u00a0be<\/em><\/strong>\u00a0signed by you. Locate the &#8220;Student Registration&#8221; card and click the &#8220;Update Personal Info\u201d link.\u00a0 Once you are redirected to the &#8216;Update Personal Information Menu,&#8217; click the link &#8220;Update Address(es) and Phone(s)&#8221;.
