Faculty and Staff Frequently Asked Questions
Yes. If you believe student misconduct has occurred at any level, you must notify the OSCP. Please submit an incident report or complaint regarding a student of concern through our online form:
https://westernu.az1.qualtrics.com/jfe/form/SV_3WMKF0E0tJJVUBo
There are certain conduct incidents outside of the jurisdiction of the Office of Student Conduct. If the concern/incident falls within the categories below, report them as instructed below:
- Concerns or incidents involving or related to a student club or organization: Please contact the Student Life and Student leadership office
- Concerns or incidents involving or related to employee misconduct, please contact the Office of Human Resources
- Concerns or incidents involving research misconduct or non-compliance, please contact the Office of Research Regulatory Affairs
- Concerns or incidents involving discrimination, harassment, retaliation, including gender or sexual misconduct, and hazing, please contact the Office of Title IX and Equal Opportunity.
If you are unsure whether it is a conduct issue, please contact the OSCP and we can advise accordingly.
We often reach out to connect with anyone who has submitted a report to gather more information about the concern, identify support strategies, and collaborate on informal resolutions as appropriate. However, due to FERPA regulations, we may not be able to follow up with you about your report or provide specific information about the resolution of the incident. Rest assured, if you receive the confirmation message from us, your report has been submitted, and someone will be investigating and addressing the concern according to our practice, policies, and procedures.
Referrals to OSCP cannot be submitted anonymously since it is a student respondent’s right to question a witness if the case is referred to a conduct proceeding. That being said, some acts of cheating can be verified using multiple sources like camera footage and proctor notes etc. Go ahead and submit a report but also encourage the student to submit one if they feel comfortable.

The institutional requirements are found in the current University catalog. College level standards may be defined in your college catalog, college policies, including course syllabi. Students are required to make themselves aware of the Standards of student conduct, academic integrity and professionalism contained in these items.
In addition to incidents that occur on university property or at a university-sponsored activity, the University’s standards of student conduct, academic integrity and professionalism may also be applied to your actions or behaviors that take place off campus, especially if that behavior has a significant adverse impact on the university community, its members, and/or the pursuit of its mission and educational objectives.
The Office of Student Conduct and Professionalism (OSCP) will notify you of any alleged violations, notifications of hearings and sanctions via your WesternU email address.
Once a report is received, the OSCP reviews the information provided and may request additional details from the individual who submitted the report or from other University or college offices to better understand the concerns.
Based on the nature and circumstances of the reported concern, the student may be:
- Referred to their college for educational intervention and advisement; or
